Our History
The Veterans Administration received the St. Albans Extended Care Center, now known as the VA St. Albans Community Living Center, from the U.S. Navy on March 14, 1974. The Naval Hospital was built by the Department of Defense on grounds previously occupied by the St. Albans Golf Club and Community Center. When constructed in 1950, the 3000 bed Naval Hospital comprised a vast network of 76 hospital wards connected by a corridor slightly over a mile long. The first Veteran resident was admitted on April 29, 1974. By June 1974, 63 nursing care beds were occupied and outpatient care services were offered.

Where We Are Now
The St. Albans Community Living Center serves the metropolitan area with specialized geriatric care. The Center's services include extended care rehabilitation, psycho-geriatric care, general nursing home care, and domiciliary providing psychosocial and independent living skills rehabilitation. Outpatient services have been offered at St. Albans for over 20 years.

Outpatient Specialty Services
In addition to nursing home care, St. Albans provides outpatient specialty clinics to our Veterans in the care areas of Primary Care, Optometry, Podiatry, Dermatology, Urology, Audiology & Speech Pathology, Mental Health, Physical Medicine & Rehabilitation, Prosthetics, Women's Health and an Adult Day Care Health Program.

About the Adult Day Care Health Program
Our Adult Day Health Care Program helps elderly veterans who are ready for discharge from the hospital or nursing home but are in need of continuing medical and/or rehabilitative services; we also serve Veterans who live at home but who require additional health and social services in order to avoid premature nursing home placement. This includes separate programming for veterans with Dementia. To make a referral, please call 718-526-1000, extensions 2536, 2530 or 2529.

Homeless Veterans Treatment Program
The Homeless Veterans Treatment Program at the VA New York Harbor Healthcare System provides a continuum of care which includes outreach, case management, a drop-in center, residential treatment, supported work therapy and either transitional or permanent housing.

Mental Health
The Mental Health Intensive Case Management Program (MHICM) purpose is to help Veterans who have had a history of mental illness to maintain themselves in the community to the best of their ability.

Pharmacy
The St. Albans Community Living Center pharmacy processes thousands of prescriptions daily. Many of these prescriptions are filled by the Central Mailout Pharmacy (CMOP). Prescriptions may be refilled by phone or the Internet. A Pharmacist is on duty 24 hours a day, 7 days a week. The pharmacist reviews and prepares all medications as physicians order them.

Readjustment
Our experience tells us that returnees from different war theaters have unique needs and interests. Your St. Albans Community Living Center Veterans Readjustment Office stands ready to serve you.

Visual Impairment Services
The NY Harbor Healthcare System's Visual Impairment Services Team Program (VIST) helps legally blind Veterans and their families adjust to severe vision problems. The program objectives are to identify and inform eligible Veterans about services and benefits; to ensure that health care and rehabilitation services to legally blind veterans are made available; and to help these veterans cope with vision loss.

Women's Health
Our woman Veterans' health program includes a number of clinical services available to women, including gynecology, mammography, reproductive care, and menopause treatment.
